Who is responsible for the things that happen during fast fashion

I feel like the clothes designers are responsible for what happens. I feel like they are responsible because they should be able to make sure that everything is good before selling. For the pollution part I feel like the factories are responsible for it. Overall I feel like there are more advantages than disadvantages. I say that because you could get your clothes fast and the prices are low while a disadvantage could be your clothes come damaged. I feel that there are ups and downs to fast fashion, but it's worth it.
